In this role, you’ll deliver high-quality, preventive-focused care through telehealth. ### What You’ll Do - Provide routine telehealth visits with Medicare patients (E/M and Chronic Care Management) - Create and manage personalized care plans for ongoing conditions - Collaborate with our integrated care team (nurse case managers, NPs, CHWs) - Ensure thorough documentation in our EHR and compliance with quality measures - Engage patients in preventive care to reduce avoidable complications **Why Belle, Why Now** **Healthcare is shifting homeward—and seniors deserve better.
