At the time of an offer, determination of your base pay will reflect your skills and experience as it relates to the position.*** The Social Worker provides social services to resident/families at the time of admission, during their stay at the nursing home, prior to discharge, and provides timely follow-up services in the community. ## Requirements **Essential Duties and Responsibilities:** Ø Assists in the application process by sending and receiving applications. Ø Will contact prospective resident or responsible party. Ø Contacts social services representatives and families. Assists residents to exercise their rights as citizens. Ø Develop a social history on each new resident. Ø Document, when necessary, in resident’s medical records. Ø Participate in regular quality assurance interdisciplinary team meetings. Ø Identify problems and develop and implement studies relative to issues affecting residents. Ø Propose recommendations resulting from quality assurance data. Ø Work with the interdisciplinary team and administration to promote and protect residents’ rights and the psychosocial well-being of each resident. Ø Provide time-limited counseling services to residents as deemed appropriate. Ø Discharge planning and referrals to community resources, as appropriate. Ø Will assist in admissions during an absence of the Director of Admissions. Ø Assembles admission packets. Ø Assists with all pre-admission paperwork and forwards to appropriate party. Ø Assures that all information is complete on admission record. Ø Assists in orienting resident to facility. Ø Will carry out other duties, as they relate to social services, as needed. **Departmental and Campus Expectations of Employee:** Ø Approaches people in a manner which creates harmony and promotes cooperation.
