As we continue to expand, we are currently seeking for a dedicated and experienced Registered Nurse. To join our healthcare team and drive our managed care initiatives. **JOB SUMMARY:** The registered nurse plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. **Essential Job Functions & Responsibilities Patient Care:** - Conducts initial and ongoing patient assessments, including physical exams and medical history. - Develops, implements, and revises care plans based on patient needs. - Administers medications and treatments as prescribed by physicians. - Provides direct patient care, counseling, and health education. - Coordinates discharge planning and necessary rehabilitative care. - Serves as Case Manager when assigned, ensuring seamless patient care. **Communication:** - Prepares clinical notes and updates physicians regularly. - Reports patient condition changes and obtains necessary physician orders. - Collaborates with healthcare professionals to coordinate care. **Additional Duties:** - Participates in on-call duties as required. - Ensures necessary medical equipment and services are arranged for patient care. **POSITION QUALIFICATIONS** - Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting and home setting. - Current licensure in state and CPR certification. - Bachelor’s degree, with one (1) year of home health care experience preferred. - Management experience not required. Responsible for supervising home health aides. - Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. - Prolonged or considerable walking or standing. Able to lift, position or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling or crouching.
