Occupational Therapist – Reablement

An opportunity has arisen for an Occupational Therapist to join our client at the Richmond and Wandsworth Reablement Service, supporting adults (18+) to regain independence, improve wellbeing, and achieve meaningful outcomes. You will manage a caseload requiring assessment, review, and ongoing support, completing weekly home visits and working closely with residents, families, carers, care agencies, social work teams, and hospitals. What will your responsibilities be? Deliver reablement‑focused OT assessments and interventions Complete Care Act assessments, Mental Capacity Assessments, carers’ assessments and reablement reviews Undertake Moving & Handling risk assessments and create Personal Handling Plans Prescribe and recommend equipment and minor adaptations Maintain accurate case records on Mosaic (daily updates required) Work in line with Care Act 2014 and local authority statutory duties Benefits: £32 per hour Supportive, dynamic team Strong supervision, CPD and development opportunities Working Pattern: Standard 36 hours, Monday–Friday Optional overtime/weekend work available Hybrid role: council offices for visits, home‑based admin and case management Qualifications & Experience: To be successful in this role you must have, HCPC‑registered Occupational Therapist Strong knowledge of Adult Social Care legislation and OT practice Experience achieving positive outcomes through MDT working Excellent report writing and time management skills Reablement and/or local authority experience desirable Why Liquid Personnel?
