Occupational Therapist – Adults Social Care and Reablement

Liquid Personnel is looking for experienced Occupational Therapist to support its client's Adult Social Care service tea in Rutland, delivering high-quality interventions to enable individuals to remain independent in their own homes. This is a varied role, combining reablement, discharge support, and community-based interventions. What will your responsibilities be? Complete holistic, strengths-based assessments and develop personalised reablement plans Support individuals on Discharge to Assess pathways, enabling safe return home Work with community patients to prevent hospital admissions Assess and provide equipment and adaptations (including major adaptations) Complete risk assessments, including complex moving & handling cases Promote and support use of assistive technology Work collaboratively with health and social care professionals Maintain accurate case records and monitor support plans Supervise OT Assistants and provide guidance to colleagues Benefits: Competitive rate of £32 p/h Hybrid working model Broad caseload across reablement and discharge work Supportive council environment Strong likelihood of extension Qualifications & Experience: To be successful in this role you must have, Degree/Diploma in Occupational Therapy HCPC Registration Experience within Adult Social Care or Health settings Understanding of: Care Act 2014 Safeguarding procedures Strong assessment, risk management, and care planning skills Key Skill: Ability to manage and prioritise workload effectively Strong communication and partnership working Confident in moving & handling assessments IT literate (case management systems, email, data input) Working Pattern: Hybrid working: 2 days office-based 1 day working from home Remaining time in the community Why Liquid Personnel?
