The LVN HC maintains a clean, organized, and safe environment. Ability to handle a diverse range of situations and responsibilities is the primary duty of an LVN HC Including but not limited to the planning and monitoring of in-home services and for carrying out tasks in the participants' residences as assigned such as home safety assessments, data gathering and supervisory visits. **Essential Job Functions:** Duties include but are not limited to: - Participate in IDT and Care Plan Meetings as scheduled. - Must be approachable and able to communicate effectively with participants in and around the clinic area. - Assist in prepping patients for assessments or procedures during clinic or home visits. - Must be able to complete a comprehensive assessment of the participant during pre-enrollment encounter, semi-annual, unscheduled and annual assessments. - Perform and complete Home Care assessments as scheduled per policy guidelines. - Follow workflow process upon completion of Home Care assessments. - Collaborate with Home Care Coordinator – Licensed Vocational Nurse Lead (HCC LVN Lead) and Personal Care Assistant Supervisor to ensure that participants are allocated enough time and services based on assessments. - Involved in the development and implementation of Home Care Planning activities. - Must be able to complete all assessments and care plan assignment(s) and other tasks in a timely manner prior to deadline. - Must be able to effectively relay and communicate instructions from the physician to a patient or authorized person with or without use of language line or interpreter as needed - Document patient concerns, change of condition (COC) and any changes in participants’ baseline status to appropriate staff daily. - Review, follow up and document any follow-ups and Service Delivery Requests in a timely manner per policy guidelines. - Must be able to provide support to participant needs with ADLs and iADLs as needed. - Enter and concurrently acknowledge pertinent patient information, results and collected data into electronic health record system. - Ensuring patients and their families understand discharge or after visit instructions. - Ensure proper body mechanics and patients’ safety with ambulating/transfer, etc - Must show competency on using AED and perform resuscitating efforts as needed. - Attend and participate in staff meetings, in-services, projects, and committees as assigned. - Adhere to and support the center’s practices, procedures, and policies including assigned break times and attendance. - Accept assigned duties in a cooperative manner; and perform all other related duties as assigned. - The LVN HC may also be asked to perform other clerical duties as needed and requested to su…
