[Licensed.Practical.Nurse(LPN).fy26 copy.docx](blob:https://workforcenow.adp.com/c8d976be-ac65-41c8-bea2-4b3a3ea60e5e) **Job Summary:** This position is responsible for providing the highest quality nursing care to Oyate Health Center (OHC) patients. **Essential Functions:** - Understands key concepts, including same-day access, continuity of care, empanelment, integrated care, and medical home. - Works effectively in a rapidly changing and innovative health care system - Provides initial medical screening: Greets patients and directs them to the appropriate exam room. Take vital signs, basic medical history, and chief complaint, and accurately document the collected information in the Electronic Health Record (EHR). - Preforms focused assessment and/or screenings on patients as indicated for physical, mental, and psychosocial needs to develop a comprehensive care management place that will address patient’s needs. - Perform patient screenings for depression, anxiety, alcohol use, tobacco use, social determinants of health, intimate partner violence, etc., and enters them in the EHR. Notifies providers of abnormal screening results to initiate appropriate interventions and education. - Ability to recognize unusual or complex problems, screenings outside normal limits, and make proper disposition by reporting to RN Care Manager and/or Provider. - Performs basic medical procedures/treatments within the scope of practice and facility guidelines: vital signs, phlebotomy, specimen collections, EKGs, bladder scan, dressing changes, sterile field preparation, procedure assistance to the provider or RN, and draping the patient for procedures. - Appropriately prepares patient for visit and alerts provider when patient is ready to be seen. When the provider is unavailable, the LPN keeps the patient updated on the standing of their appointment, possible delays, and, when necessary, assists the patient in rescheduling or scheduling follow-up. - Review the provider's schedule throughout the day and prior to the start of the shift to ensure patient flow. - Administers prescribed medications via the following routes per scope of practice and facility guidelines: oral, vaginal, inhalations, intradermal, subcutaneous, or intramuscular route. Places peripheral IVs, administers fluids, and medications via the intravenous route according to the scope of practice and facility guidelines. - Provider patient instruction on specimen collection, procedures, and treatments per facility guidelines. - Maintains awareness of comfort and safety needs of the patient; provides instruction on procedures and treatments to the patient to gain cooperation, understanding, and allay apprehension. - Operates and monitors medical equipment in accordance with established procedures and scope of practice. - Assist the provider and the RN Care Manager in managing patients. - Coordinates with other team members to ensure the efficient and effective flow of patients through the department - Mentors and orients new staff members. - Completes daily quality control testing and documentation of blood glucose meters, - Maintains exam rooms according to protocols: Cleans rooms between patient exams to provide a clean, safe, and organized environment for the patient in compliance with infection control standards; ensures all necessary supplies are stocked and equipment is available for use. - Provides and maintains accurate documentation in the patient's EHR. - Performs the job with minimum supervision - Utilizes skills in prioritizing needs, problem-solving, and decision-making in a clinical setting. - Efficiently and effectively manages multiple responsibilities. - Maintains quality, safety, and infection control standards. - Operates and monitors medical equipment in accordance with established procedures and scope of practices. - Record accurate, timely, and appropriate information in the patient’s EHR: Document observations, nursing interventions, therapeutic measures, an…
