## Physical Therapist (PT) - Full Time **Inspired Home Health Care Agency | Maricopa County, AZ** **Inspired Home Health Care Agency is rapidly expanding, and we are hiring immediately!** We are currently seeking a **dedicated and compassionate Full-Time Physical Therapist (PT)** to join our growing home health team. The Physical Therapist will serve as a patient's partner throughout the journey of restoring mobility, improving function, and maximizing overall quality of life. Our therapists play a critical role in helping patients reach their highest level of independence through **promotion, prevention, diagnosis, treatment, and rehabilitation**. ### Compensation ### **Salary:** OR **Pay-Per-Visit Rate Available** ### Service Areas We are currently seeking therapists to service the following areas: • West Valley • Paradise Valley • North Scottsdale • Central Phoenix • Peoria • Glendale ### Why Join Inspired Home Health Care? ✔ Rapidly growing agency with strong leadership ✔ Clinician-focused and supportive culture ✔ Flexible scheduling and autonomy ✔ Opportunity to grow with a fast-expanding organization ✔ Meaningful one-on-one patient care in the home setting ### Key Responsibilities **Patient Assessment & Evaluation** - Conduct initial **OASIS evaluations and in-home assessments** to determine physical therapy needs - Review patient medical histories, diagnoses, and physician orders - Identify patient-specific goals and develop individualized treatment plans **Treatment Planning & Implementation** - Develop and implement treatment plans to restore mobility, strength, and functionality - Utilize various therapy techniques including therapeutic exercise, manual therapy, and adaptive equipment - Provide cost-effective treatments that improve motion, mobility, and independence - Reduce reliance on medications and provide non-surgical treatment alternatives when appropriate **Patient Education** - Educate patients and caregivers on exercises, recovery techniques, and safety measures - Provide guidance on proper body mechanics and injury prevention - Develop home exercise and wellness programs tailored to each patient's needs **Progress Monitoring** - Monitor patient progress and adjust treatment plans as necessary - Communicate regularly with physicians and healthcare team members regarding patient progress **Collaboration** - Work closely with nurses, occupational therapists, physicians, and other healthcare professionals - Participate in interdisciplinary care coordination to ensure comprehensive patient care **Documentation & Compliance** - Maintain accurate and timely patient documentation - Ensure compliance with **HIPAA regulations and agency policies** - Complete all documentation required for billing and insurance purposes ### Qualifications - Active **Arizona Physical Therapist (PT) License** - **Home Health experience preferred** - Strong clinical assessment and communication skills - Ability to work independently in a field-based environment - Passion for delivering **patient-centered care** ### Travel This position requires **travel to patient homes throughout assigned territories** and the ability to manage a mobile caseload. ### Benefits Medical, Dental, Vision
