Responsibilities include providing nutrition education, nutritional assessments, breastfeeding education and counseling, and individualized nutrition services in accordance with the United States Department of Agriculture (USDA) Women, Infants, and Children (WIC) Program policies and procedures. This position serves as a resource for referrals to family planning, immunization, and other Health Department and community programs and services while collaborating with an interdisciplinary team to improve the health and well-being of women, infants, and children. The Float Nutritionist will travel throughout the district to provide coverage at multiple health department locations as needed. **Work Schedule:** Full-time (40 hours per week) **Travel:** Frequent travel throughout the Northeast Health District is required to provide onsite clinic coverage. **Job Responsibilities** - Conduct initial WIC nutrition assessments and develop individualized nutrition care plans using client-centered goals. - Monitor client progress and update nutrition care plans as appropriate. - Provide medical nutrition therapy as prescribed through referrals. - Provide individual and group nutrition education, including breastfeeding education and support. - Conduct nutrition counseling for high-risk WIC participants. - Educate pregnant women, new mothers, and families on nutrition, breastfeeding, infant feeding, healthy lifestyles, and parenting-related nutrition topics. - Monitor the growth and development of infants and children and coordinate referrals to appropriate healthcare providers or specialists when indicated. - Coordinate client services and referrals with healthcare providers, community agencies, and other Health Department programs. - Participate in interdisciplinary team meetings and case reviews. - Develop, review, and/or approve nutrition education materials. - Conduct community nutrition assessments and assist with identifying local nutrition needs. - Maintain accurate client documentation in accordance with USDA WIC Program policies and procedures. - Perform other duties as assigned in support of the WIC Program.
