We are looking for a Director of Provider Network Development to join our team! You’ll be responsible for expanding our primary care network throughout Illinois (Chicago, the surrounding metro area, and into Southern Illinois) while contributing to the continued refinement of our central growth platform. Experienced in both creating and executing growth strategies, you will be comfortable driving provider growth, bringing a well-established network of physician relationships, deep knowledge of the local healthcare community, and the desire to win! Reporting to the Senior Director of Network Development, this is an opportunity for someone who is excited by building and passionate about solving tough problems with high impact. What You’ll Do As Director of Provider Network Development at Diverge, you will be accountable for supporting the ongoing refinement of our in-market growth aligned with our market health plan partners to target and contract with primary care practices that support the Medicaid population, as well as the potential to support other lines of business as applicable. Delivering this work, you will balance being hands on with initiating, negotiating, and closing contracts with independent primary care practices, while advancing partnership discussions with aggregating provider entities in the market (i.e., FQHCs, System CINs, etc.). You will drive value prop refinement, clearly articulating the differentiation of the Diverge Health model and its benefits and how it will be operationalized. The outcome will be a seamless transition of new partnerships into our network, and consistent delivery of our market growth goals. Key Areas You’ll Add Value: As an individual contributor, you will be accountable for driving the growth of our Illinois market physician partners and will carry an individual growth expectation. Identifies, targets, and cultivates relationships with independent primary care practices and aggregating provider entities with higher density of Medicaid panels to expand our provider network, aligned to our health plan partner goals and areas of desired focus. Initiates, negotiates, and finalizes contracts with primary care practices, ensuring mutually beneficial terms and adherence to regulatory requirements. Partners with our operations and population health teams to ensure a smooth onboarding process for new providers, aligning messaging and ensuring that expectations match the experience. Oversees the data exchange process between our organization and partner practices to support informed decision-making and seamless patient care coordination. Assists in the transition of relationships to our market field teams, providing essential guidance and support to maintain and grow partnerships. Manages and conducts effective and efficient contract negotiations, and develops strategies, tactics, and methods for specific network development initiatives. Owns the successful implementation and execution of short- and long-term growth strategy plans within the market, including plans based on practice size, geography, and alignment with our patient support model that adhere to deadlines.
