Advocates for and supports the client, engages with community agencies/health care providers and others on his behalf to ensure access to services needed to increase wellness self-management and reduce emergency room visits and/ or hospitalizations. Provides clinical support to the Team by providing consultation, education, information around psychosocial and/or substance abuse conditions, interventions, resources to maintain focus on outcomes and best practices. **Responsibilities** - Work collaboratively with clients and their families to provide supportive services and coordinated care. - Conduct monthly home visits to assess needs, provide support, and ensure continuity of care. - Complete health risk assessments to identify individuals who may benefit from supportive services related to birth equity, social determinants of health, and care coordination. - Connect clients to appropriate community resources and enhanced social support, including childbirth education classes, workshops, and other perinatal services. - Assist clients in developing individualized birth plans and postpartum care plans. - Facilitate access to parenting education and support services, including: - Parenting classes or workshops - Doula support services - Peer support groups and clinical support groups such as postpartum support groups - Breast education and support - Provide culturally appropriate and understandable health education on perinatal topics using techniques such as the teach-back method to ensure comprehension and informed decision-making. - Deliver patient engagement and educational programming, including printed and ele…
