Clinical Pharmacist Specialist (40 hr, Rotating)

Initiates and ensures the quality of pharmacy drug distribution services in all patient care areas. Assumes direct responsibility of direct medication distribution, IV admixtures, safety and efficacy of prescribed drug therapy in accordance with laws and regulations governing pharmacists and the practice of pharmacy by the appropriate state and federal agencies. **Job Description:** **Essential Core Responsibilities:** - Practices in accordance with the state and federal laws, rules, and regulations, including but not limited to Massachusetts Board of Registration in Pharmacy, Department of Public Health, the Food and Drug Administration, and the Drug Enforcement Administration. Maintains certification and credentials in accordance with the Board of Registration and adheres to institutional policies and procedures. - Reviews and verifies medication orders with consideration, including but not limited to the following patient specific parameters: diagnosis, weight/body surface area, age, allergies, organ function, laboratory values, medication duplication, drug interactions. Clarifies orders deemed to be inappropriate, incomplete, or otherwise unclear with the prescriber in a timely manner. - Documents and communicates pertinent information and discussion with the healthcare team and pharmacy staff to ensure continuity of care. - Provides drug information to physicians, nurses and other health care professionals. - Supervises the activities of students, technicians, and residents within the pharmacist area of practice, including checking medications such as compounded sterile products and prepared unit-dose medications, as needed. - Promotes a culture of patient safety by submitting medication related events and near misses. - Completes daily pharmacy surveillance tool monitoring, including follow-up and documentation, as well as other daily patient care initiatives. - Works collaboratively with all pharmacists, pharmacy technicians, and pharmacy personnel. Upholds a culture of respect and teamwork across the Pharmacy Department. **Essential Level Specific Responsibilities:** - Participates in the onboarding and training of new hires. - Precepts and evaluates pharmacy students and/or residents through core clinical rotations or learning experiences or learning experiences as well as student/resident-led quality improvement projects. - Actively participates in continuous performance improvement activities of the department, including organizing and considering suggestions on how to improve day-to-day operations. - Leads technicians in maintaining optimal drug inventory by ensuring medication availability while maximizing inventory turns. - Regularly finalizes department initiatives including but not limited to supporting formulary stewardship and drug utilization review, presenting new medication requests at Pharmacy & Therapeutics Committee, and/or development of departmental tools and resources. - Coordinates and finalizes assigned special projects on time. - Leads a specific core area of practice, possibly including IV room performance/operation (facility and personnel) or clinical area, through interdisciplinary education, ongoing specialty training and as a resource for Pharmacy staff. - Updates clinical guidelines and standard operating procedure and presents hospital wide educational initiatives. - Serves on departmental committees to improve process and quality outcomes. - Supports IRB-approved research activity, medication use evaluations, and cost-savings initiatives. - Supports drug shortage management. **MINIMUM QUALIFICATIONS REQUIRED:** • PGY 1 + 1-3 years OR PYG2 OR 3-5 years of experience in clinical specialty area required. • Active Pharmacist license in Massachuset…
