Behavioral Health Provider II

Position Function Summary The Behavioral Health Provider, a part of La Familia Medical Center’s (LFMC) primary care treatment team, identifies, triages and manages patients with behavioral health problems within the primary care setting. In addition, the Behavioral Health Provider will provide skill training through psycho-education and patient education strategies and will develop specific behavioral change plans for patient’s behavioral health protocols for target populations. Essential Responsibilities - Assists primary health care providers in recognizing and treating mental disorders, substance abuse and psycho-social problems; - Assesses the clinical status of patients referred by primary care providers through brief consultative contacts. Assesses, treats and refers pediatric, adults and geriatric patients, individuals, couples and families; - Works with the primary care team to treat and manage patients with chronic emotional and/or behavioral health problems efficiently and effectively. Provides consultations for all clinic sites assigned; - Assists in the detection of “at risk” patients and development of plans to prevent further psychological or physical deterioration; - Assists in preventing relapse or morbidity in conditions that tend to recur over time; - Develops patient care plans with patient and behavioral health team; - Teaches patients, families and direct care staff care, prevention, and treatment enhancement techniques, as well as family practice residents using BH curriculum; - Supervises behavioral health interns and behavioral health staff (LMSWs and LMHCs) obtaining independent licensure; - Notifies program directors of any identified problems related to patient services and makes recommendations for improvement; - Attends and participates in meetings and quality improvement activities as required; - Responds to patient or co-worker complaints and works toward a positive resolution of any dispute; - Serves as a member of site committees, as requested; - Attends residency IPOLS, planning meetings as requested by supervisor; - Attends provider meetings at the request of supervisor. Other Responsibilities - Conducts themself with integrity and professionalism consistent with workplace laws, and LFMC policy. Coordinates with other departments, works effectively with peers, and contributes to organization objectives. - Abides by policies and procedure, laws and regulations. - Maintains professional and technical knowledge by participating and completing LFMC training and external educational opportunities. Stays informed of policy, compliance and legal expectations affecting department. ### Job Requirements Education, Experience and Skills/Training - Master’s in Social Work (LCSW), or Master’s in Counseling Psychology (LPCC) required. Minimum three (3) years of experience in the field required. - Current NM licensure as a Licensed Independent Clinical Social Worker (LCSW), Licensed Clinical Psychologist, Licensed Professional Clinical Counselor (LPCC). Licensed Alcohol and Drug Abuse Counselor (LADAC) preferred. - Current Clinical Supervision Credential through NM Counseling and Therapy Practice Board or through NM Board of Social Work. Integrative clinical supervision credential for LPCCs, with additional clinical supervision training required by NM Board of Social Work for LPCCs, psychologists. - The BH II level clinician will provide general leadership within the BH department and LFH organization; contribute added responsibility for quality of BH pt care and services; and guide clinical development and training of supervisee(s). - In addition, must possess substance abuse treatment/counseling experience, sufficient prior work experience in progressively more responsible positions during which the skills and abilities to perform all the essential functions have been amply demonstrated at a consistently competent level.
