**Wellness Coach - Full Time – Cortez Integrated Health – Cortez, CO** **Position Summary:** The **Wellness Coach** supports patient wellness and health integration in outpatient clinics. Wellness coaches are an integral part of a patient’s Care Team and work within multidisciplinary teams and other support staff. This position is responsible for engagement of clinic patients into a variety of healthcare activities through population-based assessments and screenings and tracking patients’ follow-up and progress. Wellness coaches focus on prevention, health education, brief interventions and referral to additional care when indicated. **Qualifications:** Our ideal candidate for our **Wellness Coach** position will have their high school degree (or equivalent). A bachelor’s degree in social work, Psychology, Counseling, or equivalent field is preferred.
