Company Overview : Habitat Health was founded in 2023 to deliver fully integrated health and social care to older adults on Medicare or Medicaid who wish to remain living independently in their homes and communities. The company operates its model through the Program of All-Inclusive Care for the Elderly ("PACE") in collaboration with local community partners. More than 2.2 million Americans could benefit from being enrolled in a PACE program, yet less than 5% of eligible Americans are enrolled today.
