About the Role: We are seeking a VP, Clinical Services to serve as the senior physician leader for Form Health’s national telehealth clinic. Reporting to the COO with a dotted line to the CMO, you will lead clinical operations for a large, nationwide distributed practice (currently ~200 providers) while maintaining exceptional clinical quality, patient safety, and a positive team culture. This role is primarily focused on strategic and operational leadership and is accountable for clinical operations performance, workforce planning, and financial outcomes in partnership with Practice Operations, Finance, Product, Engineering, and People. This is a remote position that can be located anywhere in the United States. What you’ll do Lead and scale clinical operations across physicians, APPs, dietitians, and clinical support teams, ensuring outstanding, evidence‑based care and a superlative patient experience. Own clinical operating performance: SLAs, capacity, utilization, multi‑state workforce planning, and operational efficiency, in close partnership with Practice Operations and Finance. Operationalize clinical strategy and drive practice transformation (e.g., physician‑APP models, new service lines, workflow redesign) in collaboration with the COO, CMO, and Executive Leadership Team. Provide strategic guidance to Product, Engineering, and Operations on tools and features that impact clinicians and patients, aligning the product roadmap with evolving care models. Lead, coach, and develop clinical leaders and managers, building scalable leadership infrastructure and promoting clinician wellness in partnership with the Director of Wellness. Represent Form Health’s clinical organization externally (select conferences, client meetings, etc.) and support Sales and Marketing with clinical narratives and outcomes when needed. While this role will be primarily focused on leadership duties, we would expect this person to maintain a very small longitudinal patient panel. What you bring MD/DO, board certified in Internal Medicine, Family Medicine, Endocrinology (or relevant subspecialty), with licensure in good standing. ABOM certification OR strong commitment and eligibility to become ABOM‑certified after joining. 5+ years of clinical practice experience, with strong familiarity with obesity medicine and chronic metabolic disease treatment preferred. 3+ years of clinical leadership experience in a scaled, multi‑state telehea…
