Key Highlights - Weekly pay: $2,507 - $2,601 per week - Guaranteed hours: 35 per week - Contract duration: 13 weeks - Shift: 5x8 days - Start date: 08/24/2026 - Setting: Skilled Nursing Facility (travel assignment) Responsibilities - Assess residents to identify occupational therapy needs and develop individualized treatment plans to improve ADLs, mobility, and functional independence. - Deliver therapy sessions focused on upper-extremity function, cognitive skills, and adaptive equipment training; monitor progress toward goals. - Educate patients and families on therapy plans, home exercise programs, and safe activity participation; coordinate discharge planning as appropriate. - Complete timely documentation of evaluations, progress notes, and discharge summaries in the electronic medical record; ensure compliance with regulatory and facility policies. - Collaborate with the interdisciplinary care team (nursing, therapy, and physicians) to ensure a coordinated plan of care and safety for patients. - Participate in care rounds and contribute to quality improvement efforts as applicable.
