Liquid Personnel is recruiting a Transitions Social Worker for its client in Bristol to support young adults aged 16 to 25 moving from children to adult social care. What will your responsibilities be? You will hold a caseload of adults while working closely with children’s services to ensure smooth transitions. The role requires strong knowledge of key legislation and safeguarding processes, alongside collaborative multi-agency working. Key Requirements: Qualified Social Worker with Social Work England registration Strong knowledge of the Care Act, Mental Capacity Act, best interest processes and safeguarding, including s42 enquiries Experience supporting young people or adults with complex needs Desirable: Experience with DoLS, Court of Protection, CHC, SEND or EHCP Familiarity with LAS system Working Pattern: Full-time 2 days per week in the office at City Hall (Wednesday plus one other day) Home working available on other days First week fully office-based for training and induction Why Liquid Personnel?
