## Description **Job Title:** Temporary Licensed Social Worker **Location:** 315 Courthouse Road, Princeton, WV 24740 **Position Type:** Temporary Full-Time (4-month contract) **Contract Period:** Mid-September through the end of January **Job Summary:** Princeton Health Care Center is seeking a compassionate and dedicated Social Worker to join our nursing home team on a temporary full-time basis. This four-month contract will provide coverage mid-September through the end of January with the **opportunity to stay on PRN after contract period ends**. The successful candidate will support residents and their families by providing psychosocial assessments, care planning, counseling, advocacy, and coordination of services to enhance residents’ quality of life. **Requirements:** · Bachelor’s Degree in Social Work. · Valid Social Work License through the WV Board of Social Work. · Experience in long-term care, healthcare, or related. · Excellent communication, organizational, and interpersonal skills. · Ability to work independently and as part of a multidisciplinary team. **Job Duties & Responsibilities:** · Pre-admission contact and assessment, including exploration of alternative types of placement or services with both resident and/or family. · Orientation at Admission and helping to integrate the Resident and family into the Nursing Home environment. · Integration of Social Services with other services that are provided for the resident. · Provisions for counseling residents and families utilizing modern techniques and methods for interventions as well as behavior modifications, and linkage with outside agencies when needed. · Communication to other services of psychosocial factors which could affect services rendered or care delivered. · Participation in the assessment process and in development of resident care plans. · Assist Nursing Department with coordination of discharge planning as directed. · Offer support to residents and families, as well as staff, on needs related to death and dying. · Function within prescribed policies and procedures. · Function constructively under pressure and risks involved in working with persons with complex social, physical, emotional, and geriatric problems. · Responsible to receive and act on resident complaints. · Delegate authority, responsibility, and accountability to other responsible department personnel. · Review social services department work and make recommendations to the Director of Social Services.
