We want everyone to be as proud of what we are achieving – and excited about our future – as we are, and we are committed to working together to achieve this. **About the Role:** The Social Activities & Community Transport Team Leader provides leadership, coordination and supervision to the social support and community transport team as well as managing client onboarding and review processes for this program. You will be responsible for: - Provision of leadership, coordination, support and supervision to the social support and community transport staff and volunteers. - Ensuring the Social Support and Community Transport program operate effectively and efficiently through delivering a range of stimulating and interest-based activities and outings for older people. - Monitoring the ongoing health and well-being of clients and their carers and making appropriate referrals for review and/or reassessment. **Key Selection Criteria:** To be successful in this role, you will bring the following skills and competencies: - Demonstrated experience in leading teams. - Experience working with older people in community settings. - Ability to lead and motivate staff to achieve service outcomes. - Demonstrated experience in creating stimulating interest based activities. - Experience in developing service specific care plans. - Knowledge of the Commonwealth Home Support Program. **Mandatory pre-employment requirements:** - Be willing to undergo and complete a satisfactory criminal history check. - Provide or obtain an employment-based Working with Children check. **What we can offer you in return:** - Generous leave provisions, including accrued time off, parental leave, wellbeing leave, cultural and ceremonial leave, gender affirmation leave. - Dedicated Staff Wellbeing programs, events and gym membership discounts. - Learning and development opportunities so our people can grow and thrive. **How to apply:** 1.
