You will be responsible for: - Provision of leadership, coordination, support and supervision to the social support and community transport staff and volunteers. - Ensuring the Social Support and Community Transport program operate effectively and efficiently through delivering a range of stimulating and interest-based activities and outings for older people. - Monitoring the ongoing health and well-being of clients and their carers and making appropriate referrals for review and/or reassessment. **Key Selection Criteria:** To be successful in this role, you will bring the following skills and competencies: - Demonstrated experience in leading teams. - Experience working with older people in community settings. - Ability to lead and motivate staff to achieve service outcomes. - Demonstrated experience in creating stimulating interest based activities. - Experience in developing service specific care plans. - Knowledge of the Commonwealth Home Support Program. **Mandatory pre-employment requirements:** - Be willing to undergo and complete a satisfactory criminal history check. - Provide or obtain an employment-based Working with Children check. **What we can offer you in return:** - Generous leave provisions, including accrued time off, parental leave, wellbeing leave, cultural and ceremonial leave, gender affirmation leave. - Dedicated Staff Wellbeing programs, events and gym membership discounts. - Learning and development opportunities so our people can grow and thrive. **How to apply:** 1.
