Position Details Title : Team-Based Care (EMPOWR) Clinician Classification : Regular Full Time (1.0 FTE), Exempt Location : Oakland, CA / Hybrid (availability to provide client services in-person and complete documentation remotely) Regular Work Schedule : Full time, M-F Compensation: Salary for clinicians is $ 72,000 per year for those collecting hours toward licensure (Master's level), and licensed starting at $82,000. Additional 10% Spanish language differential for qualified candidates The EMPOWR Program ( EM bracing P rogress and O pportunities, W e R ise), operates on a team-based care model designed to aid transition-aged youth (ages 16-24) in securing mental health assistance, stable housing, employment and educational advancements, and fostering community connections. The program delivers community-based therapy, case management, and care coordination, alongside peer and family support, all within a cohesive team framework. By pooling resources and expertise, the EMPOWR team collectively manages a single caseload, ensuring that youth receive comprehensive support while enabling staff to navigate the successes and obstacles of their clients together. EMPOWR Clinicians deliver community-based, intensive strategies to address any current need for stabilization, crisis intervention, and individual trauma-focused treatment while coordinating care within a team-based setting. As an EMPOWR Clinician, you will provide: Community-based trauma-informed psychotherapy to foster youth or former foster youth transitioning to or struggling with independent living. Intensive mental health services and clinical case management as part of a collaborative service team. Mental health screening to assess cognitive, emotional and social functioning and needs; facilitate the identification and engagement in mental health service Risk assessments, crisis intervention, stabilization, and safety plans. Coordinated services with other agencies and resources to address youth's needs, including housing, medical, mental health, education, employment, independent living skills, advocacy, placement stability, and enrichment activities. Stabilization support services to children and TAY in an in-person milieu environment. What will you gain?
