We are currently seeking a passionate and skilled **Speech Therapist** to join our dynamic Rehab team. You\'ll provide individualized, patient-centered therapy services that help clients regain function, improve communication, and maintain independence-all while enjoying a healthy work-life balance. * * * ### What You'll Do: - Evaluate, plan, and implement speech-language pathology treatments in the home setting - Collaborate with physicians, nurses, and interdisciplinary care teams - Document progress and adapt care plans based on patient needs - Educate patients and families to support treatment goals - Provide compassionate, evidence-based care that promotes dignity and well-being - Maintain compliance with all state and agency requirements - Accurately document treatments, standardized tests, patient education, and functional progress in the EMR. - Ensure documentation reflects visit-to-visit progress or interventions aimed at preventing deterioration. - Synchronize documentation per agency standards and timelines. * * * ### What you Bring: - Current SLP license in **New Hampshire and Maine** (or ability to obtain) - Minimum 1 year of **home care** experience preferred - Strong communication and time-management skills - Passion for making a difference in the lives of patients and families * * * ### Why Cornerstone VNA?
