**Position –** Speech Language Pathologist (SLP) **Location:** Ingleside Communities – Mount Horeb, WI (on-site position) **Status:** Part-Time **Schedule:** Days \_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_ **Job Summary:** The Speech-Language Pathologist evaluates, diagnoses, and treats individuals with speech, language, communication, cognitive, voice, and swallowing disorders. This position develops individualized treatment plans, provides therapy, documents progress, and collaborates with physicians, nursing staff, rehabilitation professionals, families, and caregivers to support each patient’s functional abilities and quality of life. A recertification by physician every 60 days is necessary if further treatment is to be continued. A progress note is written each visit. A reevaluation is written when expected duration of treatment is reached. A summary is written upon patient’s discharge. - Assists the physician in evaluating level of function. - Helps develop the plan of care and revises as necessary. - Prepares clinical and progress notes. - Advises and consults with the family and other Agency personnel. - Participates in in-service programs. - Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames. - Appropriately utilizes ICD-10 codes. - Participates in QAPI activities as assigned. \_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_ **Qualifications:** Required: - A person who has a master’s or doctoral degree in Speech Language Pathology, and is licensed as a Speech Language Pathologist by the state where they furnish services, or - A person who has successfully completed 350 clock hours of supervised clinical practicum (or be in the process of completing), at least nine months of supervised full-time SLP experience, and has successfully completed a national examination approved by the Secretary. - Currently licensed in the state(s) in which practicing.
