Social Worker - Review Team

Liquid Personnel is recruiting an Adult’s Social Worker to join its client’s Review team, based in East Riding of Yorkshire. What will your responsibilities be? Complete strengths-based reviews and reassessments of adults with care and support needs, ensuring outcomes remain person-centred and compliant with the Care Act 2014. Review existing care packages, direct payments and support plans, making recommendations to promote independence, manage risk and achieve value for money. Work collaboratively with service users, families, providers and partner agencies to assess changing needs, coordinate services and ensure timely interventions.
