Social Worker (PRN) - Hospice

We are seeking a compassionate and dedicated **Licensed Social Worker** to join our team. The ideal candidate will have a strong background in **geriatrics** and **hospice care**, with experience in an **outpatient setting**. This role involves delivering a wide range of social work services to hospice patients and their families, including: - Conducting initial **emotional, spiritual, and psychosocial assessments** - Providing **ongoing counseling and support** - Offering **bereavement services** - Participating in **community education, outreach, and referrals** As a key member of the **Interdisciplinary Group (IDG)**, the Hospice Social Worker plays an integral role in assessing patient needs, developing individualized care plans, and ensuring compliance with hospice care standards. The position will cover our Ridgeland and Vicksburg, MS office location area, including the surrounding and outlying communities.
