Hold a social work degree. Right to work in the UK and hold clean UK driving license. Registered with SWE Post qualified experience within children and family services. Key Responsibilities: Conduct comprehensive assessments and create tailored transition plans. Provide guidance and advocacy to young people and their families throughout the process. Collaborate with multidisciplinary teams, including education, health, and social care professionals. Ensure compliance with safeguarding policies and care legislation. Maintain accurate records and provide detailed reports to inform decision-making. Promote independence, inclusion, and person-centered care approaches.
