This is a 6-month post aimed at addressing the backlog of Care Act reviews within the Community Learning Disability Service—currently totaling 475 outstanding reviews across three teams. This is a great opportunity for experienced professionals to make a meaningful impact in a high-demand area. What will your responsibilities be? Conduct comprehensive assessments of individuals with learning disabilities to determine their needs and eligibility for services. Develop and implement personalized care plans in collaboration with service users, their families, and other professionals. Prioritize and manage a caseload of individuals on the waiting list, ensuring timely interventions and support. Work collaboratively with healthcare providers, educational institutions, and community organizations to coordinate services and resources. Monitor and review care plans regularly to ensure they meet the evolving needs of service users. Respond to safeguarding concerns and take appropriate action to protect vulnerable individuals. Maintain accurate and up-to-date records of all interactions and interventions. Provide emotional and practical support to service users and their families. Advocate for the rights and needs of individuals with learning disabilities. Participate in regular training and professional development activities. Qualifications & Experience: To be successful in this role you must have, Bachelor's degree in Social Work or a related field. Registration with the Health and Care Professions Council (HCPC). Experience in working with individuals with learning disabilities. Strong understanding of relevant legislation and policies, including the Care Act 2014. Excellent communication and interpersonal skills. Ability to work independently and as part of a team. Strong organizational and time management skills.
