Liquid Personnel is recruiting Social Worker for its client's Learning Disabilities Team at Powys. This is an excellent opportunity for a dedicated practitioner to support adults with learning disabilities to achieve positive outcomes, maximise independence, and maintain meaningful lives within their local communities. The successful candidate will work with adults, families, carers, and partner agencies to deliver strengths-based, person-centred social work interventions in accordance with the Social Services and Well-being (Wales) Act 2014. The role involves managing a varied caseload, undertaking assessments, care and support planning, safeguarding responsibilities, and promoting the rights, wellbeing, and independence of individuals with learning disabilities. What will your responsibilities be? Undertaking strengths-based assessments in accordance with the Social Services and Well-being (Wales) Act 2014 to identify eligible care and support needs. Managing a caseload of adults with learning disabilities and complex needs, ensuring timely and effective interventions. Developing, implementing, and reviewing person-centred Care and Support Plans that promote independence, wellbeing, choice, and control. Working collaboratively with individuals, families, carers, advocacy services, health professionals, housing providers, and partner agencies to achieve positive outcomes. Undertaking safeguarding enquiries and interventions, ensuring adults are protected from abuse, neglect, exploitation, and self-neglect. Completing risk assessments and implementing positive risk management strategies that balance safety with individual choice and independence. Undertaking Mental Capacity Assessments and contributing to Best Interest decision-making processes where required. Supporting adults through key life transitions, accommodation moves, changes in support arrangements, and community integration. Promoting access to community resources, direct payments, and opportunities that maximise independence and social inclusion. Participating in multidisciplinary meetings and contributing to integrated approaches to assessment, planning, and service delivery. Maintaining accurate, timely, and professional case records, assessments, reports, and support plans in line with statutory requirements and organisational policies. Advocating for individuals and ensuring their voice, wishes, and desired outcomes remain central to all areas of practice. Qualifications & Experience: To be successful in this role you must have, Professional Qualification: Degree in Social Work, Diploma in Social Work, CQSW, or equivalent recognised Social Work qualification. Registration: Must be registered with Social Care Wales. Experience: Significant post-qualified experience within Adult Social Care, ideally within Learning Disabilities services. Legislation Knowledge: Strong understanding of the Social Services and Well-being (Wales) Act 2014, Mental Capacity Act 2005, Human Rights legislation, and Adult Safeguarding procedures. Learning Disabilities Experience: Experience supporting adults with learning disabilities, autism, and complex care and support needs. Assessment Skills: Proven ability to undertake strengths-based assessments, reviews, care and support planning, and risk management. Safeguarding Experience: Experience managing safeguarding concerns and protecting vulnerable adults through effective intervention and partnership working. Multi-Agency Working: Ability to work collaboratively with health services, housing providers, advocacy organisations, carers, and other statutory and voluntary sector partners. Communication Skills: Excellent written and verbal communication skills, including report writing, assessment work, and relationship-building.
