Liquid Personnel is currently recruiting for an experienced Social Worker to join its client's Learning Disabilities Team in Powys. This is an excellent opportunity for a skilled practitioner to support adults with learning disabilities to achieve positive outcomes, maximise independence, and maintain strong connections within their local communities. Working within Adult Social Care, you will undertake strengths-based assessments, care and support planning, safeguarding enquiries, and complex case management. The successful candidate will work collaboratively with individuals, families, carers, health professionals, and partner agencies to deliver person-centred support in line with the Social Services and Well-being (Wales) Act 2014. This role requires a confident practitioner who can manage a varied caseload, exercise professional judgement, and promote the rights, wellbeing, and independence of adults with learning disabilities. What will your responsibilities be? Managing a caseload of adults with learning disabilities, ensuring timely assessments, reviews, and interventions are completed to a high professional standard. Undertaking strengths-based assessments in accordance with the Social Services and Well-being (Wales) Act 2014. Developing, implementing, and reviewing person-centred Care and Support Plans that promote independence, choice, and wellbeing. Working collaboratively with individuals, families, carers, advocacy services, health professionals, and partner agencies to achieve positive outcomes. Identifying and responding to safeguarding concerns, undertaking enquiries and risk assessments in line with safeguarding legislation and local procedures. Supporting individuals through complex situations including transitions, accommodation issues, health needs, and community-based support arrangements. Undertaking Mental Capacity Assessments where required and supporting best interest decision-making processes. Promoting positive risk management and enabling individuals to maintain as much control and independence as possible. Facilitating access to community resources, support networks, direct payments, and preventative services. Maintaining accurate and timely case recordings, assessments, reports, and support plans in line with statutory and organisational requirements. Participating in multi-disciplinary meetings and contributing to integrated support planning and service delivery. Qualifications & Experience: To be successful in this role you must have, Professional Qualification: Degree in Social Work, Diploma in Social Work, CQSW, or equivalent recognised Social Work qualification. Registration: Must be registered with Social Care Wales. Experience: Significant post-qualified experience within Adult Social Care, preferably supporting adults with learning disabilities. Legislation Knowledge: Strong understanding of the Social Services and Well-being (Wales) Act 2014, Mental Capacity Act 2005, Human Rights legislation, and Adult Safeguarding procedures. Learning Disabilities Experience: Proven experience working with adults with learning disabilities and supporting complex care and support needs. Assessment Skills: Ability to complete comprehensive strengths-based assessments, reviews, and care and support planning. Communication Skills: Excellent verbal and written communication skills, with the ability to work effectively with service users, families, carers, and partner organisations. Multi-Agency Working: Experience working collaboratively within multidisciplinary and multi-agency environments.
