In this role, you will, Conduct assessments and develop care plans tailored to individuals with learning disabilities, ensuring their needs and goals are met. Work collaboratively with families, carers, and multi-agency professionals to coordinate support and safeguard wellbeing. Maintain accurate case records, manage caseloads effectively, and ensure compliance with statutory responsibilities and safeguarding protocols.
