Our client, based in Solihull, is seeking an experienced Social Worker to join their busy Discharge to Assess Team. This is an exciting opportunity for a skilled Adult Social Care practitioner to support individuals transitioning from hospital settings back into the community, ensuring safe, timely, and person-centred discharges. Working within a fast-paced environment, you will undertake Care Act assessments, support planning, mental capacity assessments, and safeguarding work to ensure people receive the right support at the right time. The successful candidate will be confident managing their own caseload, balancing competing priorities, and working independently across a range of complex situations. A strengths-based approach and commitment to promoting independence and wellbeing will be central to your practice. Key Responsibilities Completing Care Act 2014 assessments and reviews for adults requiring support following discharge from hospital. Developing and implementing person-centred care and support plans that promote independence, wellbeing, and positive outcomes. Undertaking Mental Capacity Assessments and contributing to Best Interest decision-making in line with the Mental Capacity Act. Identifying, assessing, and responding to safeguarding concerns in accordance with safeguarding legislation and local procedures. Managing a varied caseload independently within a fast-paced Discharge to Assess service. Working collaboratively with health professionals, hospital teams, service users, carers, and community-based services to facilitate safe hospital discharges. Applying strengths-based practice to maximise independence and reduce ongoing reliance on formal services where appropriate. Undertaking face-to-face visits across Solihull and surrounding areas to complete assessments and reviews. Assessing risk and ensuring proportionate interventions are in place to support safe discharge and ongoing community living. \ Maintaining accurate and timely case recordings, assessments, reports, and support plans in line with statutory and organisational requirements. Qualifications & Experience Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW. Registration: Must be registered with Social Work England. Experience: Significant post-qualified experience within Adult Social Care, including hospital discharge, assessment, and support planning. Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Adult Safeguarding legislation, and associated statutory guidance. Assessment Skills: Proven ability to complete high-quality Care Act assessments, Mental Capacity Assessments, and outcome-focused support plans. Safeguarding Experience: Experience managing safeguarding concerns and undertaking risk assessments within adult social care practice. Skills: Excellent communication, assessment, organisation, and time management skills with the ability to work autonomously and manage competing priorities. Driving Licence: Full UK driving licence and access to a vehicle is essential due to the requirement to undertake face-to-face visits across the borough and wider area.
