## Overview **The Work:** The Social Worker position supports the home to retired enlisted veterans and exists to provide these veterans with the care they earned in recognition of their service located in Washington, D.C. The client is a small independent agency within the executive branch of the federal government that has a history of caring for American veterans for over 200 years. This position aligns with Cayuse’s core values of Innovation, Excellence, Collaboration, Adaptability, and Integrity by fostering technical solutions that meet customer needs, promoting teamwork, and prioritizing quality in deliverables. ## Responsibilities **Key Responsibilities:** - Work independently to examine current standards of care, compliance with those standards, and development of new standards as necessary to meet the changing needs of the residents and agency. - Work collaboratively with department heads to review patient care activities with individual clinicians to ensure knowledge and compliance with standards. - Actively participate in developing and maintaining public understanding and community outreach to influence public attitude and coordination with the agency. - Work with limited to no supervision, exercising independent clinical judgment within scope of practice. - Provide clear and concise instructions and health information to patients. - Maintain thorough, accurate, and timely patient records in compliance with legal, regulatory, and organizational standards. - Attend and actively participate in meetings, trainings, and briefings as requested, and contribute input as appropriate. - Prepare and submit required reports, summaries, and documentation in a timely and accurate manner as requested. - Maintain professional, courteous, and effective communication with employees, leadership, and other internal and external partners. - Demonstrate professional appearance, demeanor, and conduct at all times, serving as a positive representative. - Adhere to all AFRH policies and procedures. - Comply with employer work practices and standards, including documentation requirements, timekeeping, and performance expectations, as outlined in internal policies. - Other duties as assigned. ## Qualifications **Qualifications – Here’s What You Need:** - Master’s degree in social work from a school accredited by the Council on Social Work Education. - Must possess a current State of D.C. license to practice as an Independent Practitioner with eligibility and willingness to maintain good standing for the duration of employment. - Only Clinical Social Worker, Independent Clinical Social Worker or Certified Social Worker licenses are qualifying for this position. - Ph.D is strongly preferred but not required. - One year experience with providing admission preparation, assessment, counseling, day-to-day assistance, initiation and revision of care plans and integrated services to effectively coordinate resident/patient care within a geriatric population - Experience with long-term care and interacting with geriatric patients is highly preferred. - Substantial experience in healthcare settings dealing with geriatric population, dementia, and/or medical and psychiatric social work skills. - Ability to successfully pass a minimum Tier One background check, including initial and any subsequent or periodic reinvestigations as may be required during employment, and to meet all eligibility requirements for access to secure federal facilities and systems. **Minimum Skills:** - Ability to read, write, and speak English proficiently to effectively communicate with employees, providers, and other stakeholders, and to accurately document clinical encounters and health information. - Clear and professional communication skills in both face-to-face and telephone interactions, with the ability to convey information in an understandable manner. - Exceptional oral and written communication skills, including the ability to prepare concise documentation, reports…
