Under the supervision of the Director of Health Services, the Program Coordinator/Social Services Designee is responsible for developing, implementing and supervising a balanced program of recreational activities as well as oversight of all social service needs for residents at the Health Center in accordance with company policies and procedures and governmental regulations. **ESSENTIAL DUTIES AND RESPONSIBILITIES:** - Provides quality Customer Service efficiently to residents, families, co-workers and vendors in a manner to ensure satisfaction. · - Designs, plans and executes diversified schedules of recreational and social activities to assist residents in maintaining their physical and mental capabilities and promote whole person wellness in accordance with the different dimensions, i.e. physical, mental, social, emotional, spiritual and vocational. · - Assesses each resident upon admission for both recreational and psychosocial needs and develops an individualized plan of care to meet those needs. · - Provides for on-going activity and psychosocial assessments of each resident in accordance with MDS schedules and revises care plans as necessary to meet changing needs. · - Insures proper documentation of all assessments for each resident. · Attends and contributes to Care Conferences, Risk Management and QA process and meetings. · - Works closely with all staff to encourage support for programming and involve other staff members as appropriate. · - Work closely with residents and family members to provide psychosocial support and meet non-medical needs. **EDUCATION AND/OR EXPERIENCE:** ·Bachelor’s degree (B.A.) in behavioral science, OR minimum 2 years of experience as a Social Services Assistant providing full range of social services to residents of a geriatric facility. **BENEFITS INCLUDE:** - competitive wages, - amazing health benefits (medical, dental & vision for 32+ hrs per week) - employer paid life insurance, - a great retirement program, - holiday pay, - paid time off (or sick time), - free employee meals, - monthly team meetings and celebrations, - use of the fitness facilities.
