# Speech-Language Therapist (SLT) – PRN ## Ascend Home Health & Hospice | Northern Utah Ascend Home Health & Hospice is seeking a compassionate and motivated **Speech-Language Therapist (SLT)** to join our growing team on a **PRN basis** serving patients throughout **Cache and Box Elder Counties** in Northern Utah. This position offers flexibility and the opportunity to make a meaningful impact by helping patients improve communication, cognition, and swallowing abilities in the comfort of their own homes. ## Responsibilities: - Evaluate and treat patients with speech, language, cognitive, voice, and swallowing disorders - Develop individualized treatment plans based on patient needs and goals - Educate patients, families, and caregivers on therapy techniques and care plans - Collaborate with physicians, nurses, therapists, and the interdisciplinary care team - Complete timely and accurate documentation ## Qualifications: - Current Utah SLT license required - Home health experience preferred, but not required - Strong communication and organizational skills - Compassionate, dependable, and patient-focused - Valid driver's license, reliable transportation, and current auto insurance required ## Position Details: - PRN / Flexible scheduling - Coverage area includes Cache and Box Elder Counties - Pay per visit DOE (dependent on experience) At Ascend, we are committed to providing compassionate, personalized care while supporting our team members with a positive and collaborative work environment.
