## Description **The Harris Center for Mental Health and IDD ("The Harris Center")** is on the hunt for a dedicated Service Coordinator who is passionate about empowering individuals with intellectual disabilities. In this vital role, you'll help our clients navigate various services—from medical and psychiatric assistance to residential and financial support, and beyond. You'll serve as a critical bridge between consumers, legally authorized representatives, program providers, and other key collaborators. We're looking for someone with a strong blend of education, experience, and compassion, combined with a profound proficiency in creating and managing care plans. You'll also play a key role in assessing the present and future medical needs of our clients. So, if you’re ready to make a real difference, come join our team at The Harris Center! **Your Role in Action:** - Conduct comprehensive assessments, adhering strictly to our department's policy and procedural guidelines. - Ensure client welfare through regular, at least quarterly, monitoring tailored to each consumer's unique needs. - Maintain the highest standard of data integrity by updating consumer files as necessary. - Be responsible for timely documentation, ensuring all records are updated within our established timelines. - Uphold a safe and healthy environment by complying with all safety and health requirements and staying updated with mandatory training programs. **What qualifications you will need:** ***Education*** - Bachelor’s degree (B.A.) from four-year accredited college or university with a major in a social, behavioral, or human service field, including but not limited to: psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, and criminal justice. - OR any Bachelor's degree with one (1) year experience working with persons with intellectual and developmental disabilities ***Experience*** - Minimum of one year experience in the human services field preferred. - Experience working with persons with intellectual disabilities preferred. - Must possess effective written, verbal communication, and organizational skills. - Must be able to work autonomously in a fast-paced environment. - Effectively communicates and works with consumers, family members, visitors and fellow employees in a manner that demonstrates knowledge of cultural diversities. ***Other Key Qualifications*** - Must maintain a valid Texas Driver's License and be insurable under Agency policy. - Must have reliable transportation. **What we have to offer:** The Harris Center not only offers a competitive salary but also a suite of excellent benefits designed to enrich your life both professionally and personally.
