**GOODWILL GULF COAST** **JOB DESCRIPTION** **Title:** Service Coordinator / Special Instructor (conditional) **Date Written:** 8/2022 **Department:** Children and Family Services **Supervised by:** EI Program Coordinator **Supervises:** N/A **Status:** Non-Exempt/Full-Time **Mission Contribution:** Strengthens families by identifying and/or providing services to children ages 0-3 with disabilities and their families. **Function:** To provide services and referral information to families with children ages 0-3 years with disabilities. **Essential Functions:** (must be able to perform these functions, with or without a reasonable accommodation). 1. Conducts assessments and evaluations to determine Part C eligibility using system approved procedures and tools. 2. Monitors the implementation and delivery of program activities ensuring that all assigned children receive services as indicated on IFSP . 3. Maintains current records and ensures program compliance with State Department of Rehabilitation Services. 4. Provides and maintains proper, updated and accurate documentation on each child concerning contact with parents, other professionals and service providers. 5. Participates in state provided appraiser reviews with representatives of the state department of rehabilitation services. 6. Schedules and facilitates each childs individual family service plan meetings within the timeframe dictated by state guidelines. 7. Conducts initial intake interviews and helps identify family strengths and needs. 8. Collaborates with educational, medical and rehabilitation staff for optimal case coordination and service delivery and to obtain optimal results of early intervention services. 9. Actively participates in the Early Intervention Council. 10. Conducts evaluations and assessments and writes summaries of developmental results. 11. Implements recommendations from multidisciplinary team members into developmentally appropriate treatment activities. 12. Assists in development of appropriate goals and benchmarks for each eligible child based on current performance and profile results. 13. Plans appropriate developmental activities to target goals identified on childs IFSP. 14. Provides individualized special instruction services within the childs natural environment as outlined on the IFSP, if assigned. 15. Provides parent consultation, education and training to support the familys efforts in facilitating childs developmental progress. 16. Maintains timely and accurate documentation of session visits and other parent contacts for maximum program accountability. 17. Facilitate efficient bi-monthly staff meetings with early intervention team members. 18. Responsible for putting all required documentation in state agency database and Goodwills database within set timeframe. 19. Observes all safety procedures and personnel policies. 20. Regular attendance, as outlined in Goodwills policy, is required. 21. Other duties as assigned. **Skills, knowledge and abilities:** (these are required to enable job holder to perform the essential functions of the job). 1. Must be able to interact cordially and productively with a variety of people. 2. Must be able to market Goodwill and explain the mission to the general public. 3. Ability to use general office equipment including computer keyboard by touch. 4. Must possess good organizational and time management skills. 5. Must be able to take initiative, make decisions and work well with little supervision. 6. Must be able to keep information confidential. 7. Must be able to read, write and communicate clearly in English. 8. Must be able to maintain an effective working relationship with, and among employees and trainees, while motivating them to their best performance. 9.
