Responsibilities include coordinating care between inpatient and outpatient settings, participating in hospital rounds, conducting patient follow-up, facilitating transitions to hospice services when appropriate, providing education to patients and families, and serving as a resource for providers and clinical staff. This role combines the relationship-centered aspects of traditional nurse navigation with the unique opportunity to support patients across multiple care settings, ensuring continuity, communication, and compassionate care throughout the treatment process. At Guthrie, you're not just coordinating care—you're helping patients and families navigate complex healthcare decisions with compassion, support, and dignity when they need it most. **Position Summary:** - Serves as a patient navigator throughout the continuum of care. **Education, License & Cert:** - Diploma from an accredited school/college of nursing - Required RN licensure at time of hire. **Essential Functions:** - Functions as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers.| - Coordinates efforts for treatment and promotes quality improvement and educational efforts. - Compiles data, tracks outcomes, and makes recommendations for process improvement. - Provides referrals/support for follow-up care for patients who are uninsured or underinsured.
