RN Clinical Coordinator 2 *Full-Time - Exempt* **Job Description:** This position assumes responsibility for the coordination and delivery of nursing care and implements the policies and procedures of the facility and the nursing department. This position performs the admission assessment and accompanying documentation for all new admits to the Rehabilitation unit. In addition, completes assessments of any new admissions within the past 48 hours or patients who are at high risk for complications and maintains contact with family and patient to ensure a smooth transition to the rehab process. This position works closely with the Nurse Manager and Providers to maintain continuity throughout admission to rehab. This 24-hour management will be in accordance with federal, state, and local standards, guidelines, and regulations that govern the facility and as directed by the Director of Nursing or designee. The highest degree of quality care is to be maintained at all times. Hours are Monday - Friday, 9:30 AM - 6:00 PM. **Minimum Qualifications:** - Graduate of an accredited School of Nursing and current licensure as a Registered Professional Nurse. - Minimum of three to five (3-5) years of related experience in a healthcare facility. - BSN preferred with a minimum of two (2) years supervisory experience. - Strong verbal, written, computer and leaderships skills required.
