Manages the nursing care for an assigned caseload of approximately 15 patients with varying acuity and needs. Caseloads above this target will be co-managed or additional support will be offered. In the event the assigned caseload falls below this target, the nurse may be asked to provide support to other team members. ## **Essential Duties & Responsibilities:** - Practice within the scope of license, certification or registration, where applicable, and as required by law and regulation - Assess the patient's physical and mental health including the patient's reaction to illnesses and treatment regimens. - Record and report the results of the nursing assessment. - Plan, initiate, deliver, and evaluate appropriate nursing acts. - Teach, assign, or supervise other personnel in implementing the treatment regimen. - Collaborate with other health care providers in determine the appropriate patient and family-centered plan. - Implement the treatment and pharmaceutical regimen prescribed by any person authorized by State law to prescribe the regimen. - Provide teaching and counseling about the patient's health. - Report and record the plan for care, nursing care given, and the patient's response to that care. - Supervise, teach, and evaluate those who perform nursing functions and administer nursing programs and nursing services. - Provide for the maintenance of safe and effective nursing care, whether rendered directly or indirectly. ## ## ## **Knowledge, Skills & Abilities:** - Must be willing and able to care for patients with infectious disease processes unless approved accommodations - Knowledge of Federal, State and Local regulations and Medicare Conditions of Participation - Knowledge of HIPAA privacy rules and regulations - Knowledge of electronic medical records systems - Demonstrate basic data entry skills - Demonstrate excellent organization skills - Demonstrate excellent interpersonal skills - Demonstrate excellent written and oral communication skills - Ability to work under pressure to meet deadlines - A…
