RN Case Manager

**Registered Nurse (RN) Case Manager** **Ascend Home Health & Hospice** **Full-Time** **Position Overview** Ascend Home Health & Hospice is seeking a compassionate and skilled Registered Nurse (RN) Case Manager to join our team and serve patients across Cache Valley and Box Elder County, Utah. In this role, you will provide high-quality, patient-centered care while coordinating services to support patients and their families throughout their healthcare journey. You'll play a key role in ensuring continuity of care, improving outcomes, and making a meaningful impact in the lives of those we serve. **Key Responsibilities** - Provide comprehensive nursing care to patients in their homes and/or assisted living settings - Conduct initial and ongoing patient assessments and develop individualized care plans - Coordinate care with physicians, therapists, social workers, and other team members - Monitor patient progress and adjust care plans as needed - Educate patients and families on diagnoses, medications, and care plans - Complete accurate and timely documentation in accordance with regulatory standards - Participate in on-call rotation as needed **Qualifications** - Current, active RN license in the state of Utah (or compact license if applicable) - Strong clinical assessment and critical thinking skills - Excellent communication and organizational abilities - Ability to work independently while collaborating with a team - Valid driver's license and reliable transportation **Schedule & Benefits** - Full-time position - Competitive salary (based on experience) - Comprehensive benefits package including: - Medical, Dental, and Vision Insurance - Generous PTO program - 401(k) with employer match **Why Join Ascend?** At Ascend Home Health & Hospice, our core values-Serve, Lift, and Unite-guide everything we do.
