## Description Community Connections Inc. is looking for a **Registered Nurse** to join our Day Center team based in Fall River, MA on a relief basis. In this role you will support individuals with disabilities by overseeing all health-care services provided to consumers, providing direct care and in-service training in relevant medical and health-related issues, and coordinating medical services with each consumer’s primary physician or medical clinic. We are looking for a **Registered Nurse** with a commitment to care, integrity and serving the community. **What You'll Do as a Registered Nurse:** - Dispense and document consumer medications - Participate in individual case record reviews on all consumers’ with program’s team - Conduct medication reviews regularly - Review all pertinent consumer health concerns weekly with Clinical Team - Monitor and record vital signs and weights every six months or as needed - Contact physicians, family/community residences, guardians, health care professionals and service coordinators as necessary regarding specific consumer medical issues - All other job-related duties as assigned **What You'll Need as a Registered Nurse:** - Fully licensed with the Massachusetts Nursing Board - Bachelor's Degree from a four-year college or university preferred - 1+ year's experience in a community health setting - 1+ year’s experience in a training program for individuals with developmental disabilities - Valid driver's license, access to a motor vehicle, and satisfactory driving record - Excellent verbal communication and interpersonal skills - Demonstrated ability to work independently and as a part of a team - Strong computer and word processing skills **Who You Are as a Registered Nurse:** - An advocate for individuals with disabilities - A nurse looking for a regular schedule - An advocate for community health - An organized, self-starter Community Connections Inc. is an EEO employer, disability and veterans.
