Check out our website: [Home - Fortis](https://fortishh.com/) **Job Summary** Select Home Health is seeking skilled and compassionate **Registered Nurses** to provide in-home nursing care. This role ensures patients receive high-quality, personalized care while promoting independence and overall well-being. **Duties/Responsibilities:** - Perform patient assessments and ongoing evaluations. - Implement physician-ordered plans of care and complete required documentation (OASIS). - Provide in-home nursing care for multiple patients per day. - Educate patients and caregivers on medications, disease processes, and in-home care. - Coordinate patient services and supervise LPNs/HHAs as needed. - Communicate changes in patient condition to physicians and care teams. - Maintain compliance with regulations and agency policies. **Physical Requirements** - Ability to sit, stand, and drive for extended periods while providing in-home care. - Ability to assist and lift patients, as well as move equipment or supplies, with or without reasonable accommodation. - Regular use of a tablet for documentation, communication, and training. **Disclaimer:** *This description reflects the general nature and level of work for the Registered Nurse Position.
