**About This Registered Nurse (RN) Case Manager Position** HopeHealth Hospice & Palliative Care is seeking an experienced RN to care for hospice patients in patient homes in Northern Rhode Island. The RN Case Manager is an advocate, negotiator, and leader for the clinical oversight of the patient. The successful candidate is passionate about providing the best possible care at end-of-life and acts with a sense of urgency and a disciplined approach for the accuracy and quality of the care delivered. This is a full-time, 40-hour per week, position with 8am-4:30pm hours.
