Registered Nurse (RN) Access Center

**About This Access Center Registered Nurse (RN) Position** HopeHealth is seeking an experienced Registered Nurse (RN) for its referral center based in Providence, RI. The Access Center RN position acts as a customer service liaison between referral sources, care providers, professional staff, patients and families to ensure the smooth and timely admission of patients onto our hospice service. This is a full time 40 hours per week position, the start time for this position varies, 8-4:30, 10-6:30 rotation every third week) with a every 3rd weekend rotation. **What will you do as a Access Center Registered Nurse (RN)?** - Demonstrates commitment to patient care utilizing outstanding nursing assessment and communication skills throughout the entire referral and admissions process. - Schedules visits for referred patients within required timeframe. Provides follow up on pending patients and non-admissions.
