## Overview **Schedule :** Wednesday and Thursday, 9-5 Additional hours/days TBD/flexible ## Description Work with health care team providing care to LGBTQ unhoused youth age 16-24 receiving services from the Ali Forney Center (AFC) in Manhattan. Work in conjunction with Institute's primary care providers and the staff of Ali Forney. The nurse will be responsible for routine nursing duties including injections, outreach and care coordination with IFH and AFC staff; triage and assessment; self injection teaching; promote and coordinate access to PreP and PEP; coordinate with mental health providers; coordinate planning for patients who are aging out of care; manage Rx prior authorizations and coordination with pharmacies; supply inventory and ordering; may facilitate patient health education groups. ## Requirements - Demonstrates the ability to perform clinical/technical/service/administrative tasks. Records, reports and interprets clients’ responses and collaborate with appropriate members of the health team in the Family Practice. - Identifies those components of the nursing regimen, which may be delegated to allied nursing personnel and provides the necessary teaching and guidance. - Systematically and continuously assesses patient’s physical, psychosocial and spiritual health status and knowledge base to determine patient’s health care and educational needs. - Triages all sick and walk-in patients. - Establishes therapeutic and supportive relationships with patients and family. - Assures proper cost-effective utilization of resources. - Maintains an environment that is conducive to meeting patient care needs and that facilitates patient flow. - Promotes and provides for patient safety in accordance with state and federal guidelines. - Practices and monitors infection control. - Monitors patient flow and reassigns staff as needed. - Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area. - Participates in the development of other staff members. - Meets regulatory, licensure and annual health assessment requirements. - Identifies learning strengths and needs. - Utilizes learning resources. - Attends in-service and continuing education programs. - Obtains certification in area of practice. - Successful demonstration of competence in area of practice. - Demonstrates competence in utilizing the selected information systems. - Participates in special projects and committees. - Demonstrates commitment to continuing professional development through relevant continuing education. - Demonstrates a professional, courteous, and respectful attitude in dealing with patients, families and significant others. - Displays courtesy, tact and patience during interactions with all members of the hospital staff and extended community. - Informs and educates patient/family to enable participation in their care and decision making. - Evaluates the effectiveness of interventions and patient responses and outcomes to care. Communicates pertinent information and changes in patient status to appropriate members of the care team and other health care providers. - Effectiveness in supervising and mentoring LPNs, Medical Assistants, and Nursing students. - Understands the necessity of paperwork and fulfills obligations on time. - Adheres to and understands nursing policies and procedures. - Keeps abreast of new and changing techniques and concepts. - Contributes to a cooperative, friendly and mutually supportive work environment.
