**Summary** Eastern Nephrology is seeking a dedicated and skilled Registered Nurse to join our clinical team in North Carolina. The Registered Nurse will provide high-quality, patient-centered care, working closely with medical providers to execute individualized care plans. This role involves direct patient care for adult and geriatric populations, along with documentation and clinical support activities in line with our established protocols and standards. This position is designated as an on-site, patient-facing role. In-office presence is an essential function due to the need for real-time collaboration with providers, direct interaction with patients and caregivers, and secure handling of health data in compliance with HIPAA. **Qualification Requirements** To perform this job successfully, an individual must be able to participate in the assessment of patients; use critical thinking skills in response to events, observations, experiences, verbal and written communication, problem solving and decision making. Perform Chronic Care Management activities and assist with patient intake. 2. Obtain and record patient vitals, including blood pressure, temperature, pulse, and weight. 3. Review and document patient allergies, medications, and any over-the-counter drugs in the EMR system. - Clinical Documentation: Accurately document patient interactions, changes in care interventions, and communication with external providers in the EMR. 40% 1.
