Registered Nurse

**Registered Nurse | FT | Home Health | Nashville, TN** **All Heart Home Care** is seeking a compassionate and experienced Registered Nurse (RN) to join our home health team in the Nashville area. This full-time position provides the opportunity to deliver high-quality, patient-centered care in a home setting while working with a collaborative healthcare team. **Key Responsibilities:** - Perform in-home patient assessments and develop individualized plans of care - Provide skilled nursing services in accordance with physician orders and established care plans - Monitor patient conditions and communicate changes to physicians and the care team - Administer medications, treatments, and wound care as needed - Educate patients and caregivers on disease management, medications, and home care practices - Supervise and coordinate care provided by Licensed Practical Nurses and other clinical staff as appropriate - Ensure accurate and timely documentation in compliance with company policies and regulatory requirements - Coordinate care with physicians, therapists, and other healthcare professionals - Travel to patient homes within the assigned service area **Qualifications:** - Current Registered Nurse (RN) license in the state of Tennessee - Current CPR certification - Valid driver's license, reliable transportation, and current auto insurance - Strong clinical assessment, communication, and organizational skills - Ability to work independently and manage a patient caseload effectively - Previous home health experience preferred but not required **Benefits:** - 401(k) - Paid Time Off (PTO) - Medical, Dental, and Vision Insurance **Work Schedule:** - FT position **About All Heart Home Care:** At All Heart Home Care, we are dedicated to delivering compassionate, high-quality care to patients in the comfort of their homes.
