**About the Role** As a Referral Coordinator, you keep patients moving through their care journey. You own the referral process end‑to‑end—making sure every referral is accurate, authorized, scheduled, and communicated clearly. You’re the connector between patients, providers, and specialists, ensuring seniors get the care they need without delays or confusion. This is detail‑driven, people‑first work where your organization, communication, and follow‑through directly shape patient outcomes.
