Recovery Support Navigator

**Location:** Framingham, MA - Outpatient Treatment Program **Schedule:** 16 hours per week. Thursday and Friday (days may change depending on needs of the program) **Pay rate:** $22.90/hour **Benefits:** - Health, dental, vision insurance/additional voluntary insurances - Company-paid life insurance/employee assistance programs - Generous paid time off accrual - Tuition reimbursement/loan repayment options - 401k with company match up to 7%! - Discounts on wide array of services/entertainment nationwide The **Recovery Support Navigator** is responsible for: - Providing quality services and case management to assigned patients in accordance with the Recovery Navigator model. - Performing intake assessments on all new patients to the program and completes all associated documentation within required time frames. - Working with the patient to develop and implementing an individualized plan with goals and objectives based on identified needs. - Assisting patients with scheduling and coordinating medical and behavioral health appointments. - Coordinating care and the plan with existing providers including but not limited to behavioral health, primary care and Community Partner. **Qualifications:** - Bachelor’s Degree in human services related field or discipline - Or LADC - Or has a recognized certification and/or licensure for the target population to work with - Or sufficient experience and training in counseling and meets regulatory requirements
