# **Hospice Registered DietitianMake a Lasting Difference Through Compassionate Nutritional Care.** Join a hospice team where your expertise helps patients and families find comfort, dignity, and quality of life during every stage of their journey. The Care Team Hospice is seeking a compassionate Registered Dietitian to provide nutritional assessments, education, and support to hospice patients in their homes and/or in facilities. As part of our interdisciplinary team, you'll collaborate with nurses, physicians, and other clinicians to develop individualized care plans that enhance comfort and quality of life. **Schedule & Territory** • This is a part-time role working during normal business hours of Monday-Friday, 8:00 am to 4:30 pm • Based out of Mishawaka, IN, and will travel up to a one-hour radius **Responsibilities** • Complete nutritional assessments for hospice patients. • Develop individualized nutrition plans based on patient needs and goals. • Provide nutrition education and counseling to patients, families, and caregivers. • Collaborate with the interdisciplinary team to support patient-centered care. • Document assessments, interventions, and care plans in the electronic medical record. • Participate in care conferences and interdisciplinary team meetings. • Registered or Licensed Dietitian as required by the state of practice (or eligible for registration through the Academy of Nutrition and Dietetics). • Bachelor's degree in Nutrition, Dietetics, or a related field preferred. • Minimum one year of Dietitian experience. • Hospice, palliative care, home health, or geriatric experience preferred. • Strong communication and patient education skills. • Experience using an electronic medical record is preferred. Field Requirements • Valid driver's license. • Reliable transportation and current auto liability insurance. • Ability to travel throughout the assigned service area. • Ability to work in patients' homes and provide compassionate, patient-centered care.
